2024 OPEN ENROLLMENT DATES

OCTOBER 16—NOVEMBER 3, 2023

Open Enrollment for the 2024 benefit plan year (January 1, 2024—December 31, 2024) is almost here!

Open Enrollment Reminders

This is your annual opportunity to make changes to your benefits. This includes:

• Add, cancel, or change your coverage

• Add or remove eligible family members

• Enroll or re-enroll in the Health Care Flexible Spending Account (FSA) and/or Dependent Care Flexible Spending Accounts (DCFSA) - you must re-elect contributions annually; current elections will not rollover into 2024

 

How to Enroll:

1. Access the Paylocity login page—https://access.paylocity.com/ (Chrome browser recommended)

2. Login using your Username and Password. If you haven’t registered, select Register New User

For new users—select, I don’t have a Registration Passcode and enter your company ID:

    • 57741—Trinidad Benham

    • 80031—Honest Origins

    • 181697—Columbia Bean
Follow the prompts to complete registration

3. Once logged in, select the 3 line icon located in the top left corner of the screen

4. Scroll down and select Bswift Benefits

5. Hit the orange Open Enrollment button and make your benefit selections

6. Or you also have the option to call the Employee Enrollment Center; 5 AM - 3 PM Pacific, 6 AM - 4 PM Mountain, 7 AM - 5 PM Central, 8 AM - 6 PM Eastern.
Monday-Friday at: 1- 888-598-2040 and a Benefit Specialist will assist you with making your elections.

What's New In 2024

 

Below is a snapshot of what’s changing for your 2024 benefit plans.

 

• A new Buy-Up Dental Plan with Delta Dental — You will have the choice between 2 dental plans. This new

plan includes a larger maximum annual benefit, lower deductibles and now provides a benefit for child and

adult orthodontics.

 

Voluntary Hospital Indemnity with Aflac—This plan pays a fixed cash benefit directly to you when you

experience a covered hospital stay, for events like an inpatient procedure or childbirth. What’s even better?

• No deductible

• No network

• No precertification

• No pre-existing conditions (including pregnancy)

 

ARAG Legal Plan—This voluntary benefit is designed to save you time and money. Legal insurance will

make dealing with legal issues less expensive and a lot less stressful.

Eligibility

You are eligible for benefits if you

work 30 or more hours per week. You

may also enroll your eligible family

members under certain plans you

choose for yourself. Eligible family

members include:

 

• Your legally married spouse

• Your Common-Law spouse

• Your Domestic Partner

• Your children who are your

biological children, stepchildren,

adopted children or children for

whom you have legal custody (age

restrictions may apply). Disabled

children age 26 or older who meet

certain criteria may continue on

your health coverage.

 

 

When Coverage Begins

 

• New Hires: You must complete the

enrollment process within 31 days

of your date of hire. Coverage

is effective the first of the month

following your date of hire.

 

If you fail to enroll on time, you will

NOT have benefits coverage (except

for company-paid benefits).

 

Benefits elected are effective January 1 – December 31.

 

 

Choose Carefully!

Due to IRS regulations, you cannot

change your elections until the next

annual Open Enrollment period, unless

you have a qualified life event during

the year. Following are examples of the

most common qualified life events:

• Marriage or divorce

• Birth or adoption of a child

• Child reaching the maximum

age limit

• Death of a spouse or child

• You lose coverage under your

spouse’s plan

• You gain access to state coverage

under Medicaid or CHIP

Making Changes

To make changes to your benefit

elections, you must contact Human

Resources within 31 days of the

qualified life event (including

newborns). Be prepared to show

documentation of the event such as

a marriage license, birth certificate or

a divorce decree. If changes are not

submitted on time, you must wait until

the next Open Enrollment period to

make your election changes.

 

 

When Coverage Ends

If you terminate employment with

Trinidad Benham your medical,

dental, and vision coverage ends on

the last day of the month in which

your employment was terminated.

All other benefits end on your last

day of employment. Following your

termination, you will be eligible to

continue your medical including Carrot, dental, vision

and Flexible Spending Account (FSA)

coverage under COBRA. Information

will be sent to your home address

shortly after your termination.

 

 

 

Required Information—When you

enroll, you will be required to enter a

Social Security number (SSN) for all

covered dependents. The Affordable

Care Act (ACA), otherwise known

as health care reform, requires the

company to report this information to

the IRS each year to show that you

and your dependents have coverage.

This information will be securely

submitted to the IRS and will remain

confidential.
