Working directly with Allstate Benefits, GMS Benefits is now able to provide individual, small farm employers, with affordable, high-quality level-funded group health plans for them and their W-2 employees of just two or more total participants, even if those employees are husband and wife.
If you’re looking for an alternative to expensive traditional fully insured plans for individuals, this Level Funded Plan from GMS could be your answer. Under this plan, we put together a level funded self-insured group program, combined with stop loss insurance, so you get the benefits of self-funding but with no risk, and level month premium payments.
The advantage to our new level funded plan for small agricultural companies and farms is that no matter how many claims you have in a month, you will never pay more than your level monthly payment, AND
after all claims are paid for the year, a portion of the unused money in the claim fund is returned to your company. Think of it, with the GMS NE Farmers program, you can now get all these benefits and full group coverage with a minimum of only 2 W-2 Employees!
Our program combines the savings of a level-funded program with the stability that in the past, only traditional plans were able to offer.
With our level-funded program, your group gets to share in the profits when it has low claims.
Your group will have a maximum out of pocket cost, so you will easily know what to expect for cost.
You do not need to be an insurance professional and administer the program yourself…GMS along with Allstate will assist in administering your program

Do I Qualify?
If you have at least 2 total W-2 employees, even if they are husband and wife and you have a federal tax ID, you are eligible to apply for this new program. However, since not all groups will qualify, here are a couple other things you need to know before you apply:

Since these plans are underwritten, you and your employees will need to answer health questions.
If anyone in your group has any of the following illnesses you will not qualify for this program:
a. HIV/Aids
b. Hepatitis
c. Ongoing Cancers and/or cancers diagnosed within a year
d. Any non-diabetic injectable meds
e. Any pending transplants and/or surgeries
Our new NE Farmers Program offers you 4 convenient group insurance plans to choose from. All 4 plans are underwritten through Allstate Benefits and are part of the Aetna Choice POS2 Network. Plans 1 & 2 include co-pay inclusions prior to meeting deductibles. Plans 3 & 4 have relatively less expensive monthly payments than plans 1 & 2 but you must meet your deductible before these plans begin to pay out.

Using the in-network provider your deductibles are as follows: $3,500 Individual and $7,000 Family.
Office co-pays are $40 for PCP and $60 for Specialist. Once the deductible is met you will pay 30% of the claims until the maximum out of pocket is met. The maximum out of pocket is $8,550 Individual and $17,100 for Family. Preventive care/screenings and immunizations are no charge. Deductibles for non-participating providers are $7,000 Individual and $14,000 for Family with the maximum out of pocket of $25,650 Individual and $51,300 Family.

Using the in-network provider your deductibles are as follows: $3,000 Individual and $6,000 Family.
Office co-pays are $40 for PCP and $60 for Specialist. Once the deductible is met you will pay 20% of the claims until the maximum out of pocket is met. The maximum out of pocket is $7,900 Individual and $15,800 for Family. Preventive care/screenings and immunizations are no charge. Deductibles for non-participating providers are $6,000 Individual and $12,000 for Family with the maximum out of pocket of $23,700 Individual and $47,400 Family.
Using the in-network provider your deductibles are as follows: $5,000 Individual and $10,000 Family. The deductible must be met prior to the insurance company paying a claim. Once the deductible is met you will pay 20% of the claims until the maximum out of pocket is met. The maximum out of pocket is $6,750 Individual and $13,500 for Family. Preventive care/screenings and immunizations are no charge. Deductibles for non-participating providers are $10,000 Individual and $20,000 for Family with the maximum out of pocket of $20,250 Individual and $40,500 Family.
