Taking care of your teeth is as important as taking care of the rest of your body. That’s why Cook-Illinois Corp. continues to offer dental insurance which covers routine check-ups and additional services needed for your health.

Vision
Cook-Illinois Corp. offers a Vision Coverage benefit administered by BlueCross BlueShield. Under this plan, you may use the eye care professional of your choice. However, when you use a participating provider, you receive higher levels of coverage under the In-Network benefit structure.

Life Insurance
No doubt about it, people depend on you. That’s why Cook-Illinois Corp. offers supplemental voluntary life insurance. Employees who want to supplement their group life insurance benefits may purchase additional term coverage. When you enroll yourself in this benefit, you pay the full cost through payroll deductions.
Please remember that if you are electing anything over the Guarantee Issue amount(s) or are reapplying due to being declined the coverage in the past, you must answer medical questions for any amount elected and be approved through underwriting.

EAP - Employee Assistance Program
Cook-Illinois Corp. will continue to offer the Employee Assistance Program (EAP) through Guardian. All
employees are automatically eligible for the EAP.
With Work-Life Matters you will be able to receive expert
support services to help you and your family with a variety of day-to-day life issues such as child-care,
stress management, and much more. If you are enrolled in any Guardian product, you will be entitled to
three face-to-face visits per family member, per year with a doctoral psychologist or other behavioral health EAP professional. All employees are eligible for unlimited Telephonic Counseling by calling 800.386.7055, and a variety of resources to help with any life challenges you might be facing. Accident Insurance - Allstate
The Accident plan covers accidental injuries incurred off the job. If you purchase this insurance and get
hurt in a covered accident, Allstate sends you a check for covered services and lets you decide the best way to spend it! There are two levels of coverage on the Accident plan to choose from, a Low plan and a High
plan.
The Accident plan also includes an Outpatient Physician’s benefit. If you visit a physician for anything
OTHER than an accident, you can file a claim and receive either $75 or $100 depending on the plan you
select.
Critical Illness - Allstate
The Critical Illness plan offers coverage in a lump sum benefit at the diagnosis of a critical illness (heart
attack, stroke, cancer, etc.). You can purchase $10,000 worth of coverage – and you can use the money as
you see fit. You can also cover your spouse and eligible dependents! This plan offers a Wellness benefit. If
you are enrolled in this coverage and go in for your annual screenings (see packet for specifics) you can file
a claim and will receive a check for $50 for yourself and another $50 if your spouse is on the plan! What a
great way to reward yourself for taking part in staying healthy. Please note that if you waived this coverage
when first eligible, you will be subject to Evidence of Insurability.
